In N.M., using clinical health workers to improve Medicaid managed care

A Health Affairs article looks at how health workers  integrated with clinics can improve population health.  A Medicaid initiative in New Mexico called Integrated Primary Care and Community Support has provided crucial intensive intervention for patients with the highest and most chronic needs and highest costs (but lowest incomes)  and taught clinical health workers a lot about how to improve population health.

The article says the the program has “provided comprehensive individual and family support for all patients in poor health (not just those with adverse social determinants) and adopted a population health strategy for the entire population in which CHWs intervened at each level of care, aiming to prevent health status from worsening.”
