Chicago-based Cook County Health and Hospitals System plans to acquire local nursing homes.

Becker’s Hospital Review reports that CCHHS hopes  that expanding into nursing homes “will help better address its patients’ needs, improve continuity of care after patients leave the hospital and decrease readmissions.”

“The safety-net health system provides services across the continuum of care, including specialty and primary care, emergency, acute, outpatient, rehabilitation and preventative care. Adding nursing homes to its portfolio would extend this continuum even further,” the news service reported.

It’s all part of the national movement toward much more care integration.
