Chronic-care IT company raises more cash

MedCity News reports that ACT.md, an IT company  that aims  to  improve  care of patients with multiple chronic conditions coupled with behavioral health issues or social needs, has raised $8.4 million more in venture capital.

The new money will be used for  engineering to expand the set of experiences required for complex care management and to continue to build out protocols and pathways of care.

The idea is to provide a platform on which all members of a  care team can collaborate, thus improving transparency, making handoffs easier and reducing the risk of errors.
