Recent efforts to reduce hospital readmissions for common medical conditions have not led to an increase in death rates. In fact, they may have led to a decrease in mortality rates, says new study published in JAMA.
The steps that hospitals take to improve care coordination after discharge may explain the results. Those strategies include better preparing patients and their families for discharge, more timely and frequent follow-ups and improved communication with outpatient providers.
